This may include participate safe support team, support coordinator and relevant parties they wish to have attend this discussion.

Personalised Planning: Based on these discussions, we develop a customised support plan that aligns with participant’s personal goals and aspirations.

Ongoing Collaboration: Krystal Community Care work closely with participants and their support teams, including allied health professionals such as psychologists, counsellors, and occupational therapists, to ensure that the support plan remains relevant and effective.
